1. Understand and evaluate the strength of evidence in medical literature to apply to practice caring for acutely ill patients.
2.     Develop physical assessment and history-taking skills and apply critical thinking and decision-making to avoid unnecessary tests and procedures to develop differential diagnoses.
3.     Achieve competency in the technical skills of intubation, central line insertion, arterial lines, chest tubes, suturing, and lumbar puncture.
4.     Thoroughly and accurately document a patient’s history and physical, SOAP note, and progress note in the medical record.
In ANP-650, my objectives are focused on building a strong foundation to carry through this program and on into my career as an Adult-gerontology Acute Care Nurse Practitioner (AGACNP). With each patient encounter, the amount of research through medical literature is going to be extensive. To ensure high-quality evidence-based care, understanding the strength of evidence, and applying it to the patient is necessary. When having to make clinical decisions understanding information coming from a systematic review including a meta-analysis of randomized controlled trials (RCTs) is considered the stronger evidence compared to a case-control study, case report, or an expert opinion making it more reliable information to justify clinical decisions (McKean et al., 2017). During the introduction into clinical practice, I find it important to know the different levels of evidence and apply the proper weight to the information as it is being used to guide your plan of care.

As AGACNPs, it is important to accurately diagnose a medical condition or disease to provide the proper treatment in a timely manner. Laboratory tests and diagnostic imaging are an important part of excluding or confirming to reach a diagnosis. In my professional observations, some providers looking for answers may cast a wide net of testing letting the result of the tests give them the diagnosis without thoroughly assessing or interviewing the patient. This method can be effective in achieving an answer, it can also cause unnecessary discomfort and potential harm to the patient and waste resources. Unnecessary testing can be defined as tests that are ordered or repeated that do not add extra value to the medical management of the patient. These additional tests create additional financial burdens, waste resources, and can cause additional stress and discomfort to the patient (Bai et al., 2020). It is my goal to be able to incorporate critical thinking in my patient interviews and assessment to help minimize the need for unnecessary testing.
Being able to perform the procedural skills listed in object three is necessary for monitoring and administering life-saving resuscitative measures. While in practice, striving for basic competency has to be achieved before a practitioner can advance to proficiency and eventually achieve an expert skill level.
Patient documentation in the medical record serves many purposes. It facilitates communication with the patient and their healthcare team, can protect providers from legal claims, and justifies proper reimbursement for the provider’s services. In a study on medical students documenting simulated patients, ninety out of the ninety-one charts were downcoded due to incomplete documentation in the medical record which resulted in reduced reimbursement rates. The most frequent areas of missed documentation were in the review of systems, history or present illness, and past medical history (Hoonpongsimanont et al., 2018). Thorough documentation is a skill that needs to be developed through practice and repetition.  The goal is to practice documentation until all components of the medical documentation are second nature.
