Collaborative Learning Community : Technology in health care has taken many forms over the years

Technology in health care has taken many forms over the years. In 1796, Edward Jenner developed the first smallpox vaccination method. A hundred years later, Wilhelm Rontgen discovered how to use x-rays for medical imaging. And in 2013, Japanese researchers grew the first human liver from stem cells. What is next?  How does one technological advance set the stage for further discoveries?

Explain how the patient, family, or population problem impacts the quality of care

Explain how the patient, family, or population problem impacts the quality of care, patient safety, and costs to the system and individual.

    • Cite evidence that supports the stated impact.
    • Note whether the supporting evidence is consistent with what you see in your nursing practice.
    • Explain how state board nursing practice standards and/or organizational or governmental policies can affect the problem's impact on the quality of care, patient safety, and costs to the system and individual.
      • Describe research that has tested the effectiveness of these standards and/or policies in addressing care quality, patient safety, and costs to the system and individual.
      • Explain how these standards and/or policies will guide your actions in addressing care quality, patient safety, and costs to the system and individual.
      • Describe the effects of local, state, and federal policies or legislation on your nursing scope of practice, within the context of care quality, patient safety, and cost to the system and individual.
    • Propose strategies to improve the quality of care, enhance patient safety, and reduce costs to the system and individual.
      • Discuss research on the effectiveness of these strategies in addressing care quality, patient safety, and costs to the system and individual.
      • Identify relevant and available sources of benchmark data on care quality, patient safety, and costs to the system and individual.

      Care Coordination Presentation to Colleagues

Your preceptor has been observing your effectiveness as a care coordinator and recognizes the importance of educating other staff nurses in care coordination. Consequently, she has asked you to develop a presentation for your colleagues on care coordination basics. By providing them with basic information about the care coordination process, you will assist them in taking on an expanded role in helping to manage the care coordination process and improve patient outcomes in your community care center.

You are encouraged to complete the Vila Health: Cultural Competence activity prior to completing this assessment

You are encouraged to complete the Vila Health: Cultural Competence activity prior to completing this assessment. Completing course activities before submitting your first attempt has been shown to make the difference between basic and proficient assessment.

In this assessment, you will evaluate the preliminary care coordination infographic you developed in Assessment 2 using best practices found in the literature.

To prepare for your assessment, you will research the literature on your selected health care problem. You will describe the priorities that a care coordinator would establish when discussing the plan with a patient and family members. You will identify changes to the strategy based upon evidence-based practice and discuss how the strategy includes elements of Healthy People 2030

Explain how you could apply implications from the study to your nursing practice.

In 2–3 pages, not including title page and references, address the following:

          • Identify your selected article. Explain what characteristics make this a meta-analysis.
          • Were the inclusion and exclusion criteria clearly stated? How were the articles that were included selected? Do you agree with the researchers’ approach? Explain why or why not.
          • Do you agree with the conclusions? Explain why or why not.
          • Explain how you could apply implications from the study to your nursing practice.

Nutrition is the cornerstone of health and healing.

            Introduction

Nutrition is the cornerstone of health and healing. Nurses play a vital role in assessing patients’ nutritional status, identifying risk factors for dietary disorders, planning interventions, and evaluating outcomes. Nurses must integrate nutritional knowledge with clinical judgment to support optimal outcomes when caring for patients with chronic illness, hospitalized malnutrition, or those on therapeutic diets.

In this discussion, you’ll explore how to assess nutritional needs, identify at-risk clients, and propose evidence-based nursing interventions that promote comfort, health, and healing.

???? Case Scenario: Complex Nutrition Risk

Patient: Ms. Julie Green, a 71-year-old retired schoolteacher

Medical History: Congestive heart failure, type 2 diabetes, osteoarthritis

Social History: Lives alone, limited mobility, relies on frozen meals

            Recent Labs and Vitals:

            • Hemoglobin: 10.1 g/dL (low)
            • Albumin: 2.9 g/dL (low)
            • BMI: 18.3 (underweight)
            • Weight loss: 12 lbs in the last 2 months
            • Reports fatigue, poor appetite, and occasional dizziness
            • Assessment: Pale, frail appearance; reports skipping meals and limited grocery access.
            ???? Discussion Prompt

Choose ONE of the following categories and provide a detailed post:

            1. Nutritional Risk and Assessment
            • What data would you collect to assess Ms. Green’s nutritional status?
            • Based on Dudek Ch. 15, what criteria suggest malnutrition, and how would you classify her risk level?
            • What role does the nurse play in screening and early intervention?
            2. Interventions to Promote Nutritional Health
            • Propose three nursing interventions that address Ms. Green’s nutritional concerns (Taylor Ch. 37, Dudek Ch. 16–17).
            • How can these be adapted to her age, mobility limits, and chronic conditions?
            • How would you evaluate if your plan was successful?
            3. Alterations in Nutrition and Complications
            • Which nutritional complications is Ms. Green at risk for, and why?
            • How do changes in protein, fluid, or calorie intake affect patients with chronic illness or aging?
            • What interdisciplinary referrals (e.g., dietitian, social services) might be needed?
